Helen Crunk is the owner of Helen Crunk Consulting and Leven Senior Healthcare. She specializes in facility management in assisted living and direct care staff education. Prior to starting her own business, Ms. Crunk worked as an administrator and assisted living consultant in the Omaha metro area for 15 years. She was elected to the National Center for Assisted Living’s Board of Directors as an At-Large Member in September 2011 and currently serves on its Executive Board of Directors.
Ms. Crunk is a registered nurse with more than 20 years of experience in long term care. She is the only two-time winner of the NCAL Administrator of the Year Award. As an NCAL Board member, Ms. Crunk served as Chair of its Finance and Policy Committee and Quality Committee. She currently co-chairs the Independent Owner Council and sits on the AHCA Business Management Committee. In 2012, she was appointed to the American Health Care Associations Clinical Practice Committee and served on AHCA’s Quality Cabinet. In addition to these commitments she has testified as an expert witness in assisted living litigation and spoken to many state associations about quality in assisted living and dementia care best practices. Helen was a previous task force member of the National Association of Board of Examiners for long term care administrators.
Helen is Immediate Past Chair of the Nebraska Assisted Living Association, sits on the Nebraska Health Care Association Executive Board of Directors, and is secretary to the Nebraska Health Care Foundation’s Board of Directors.
In 2011, Helen was named Nebraska Business Woman of the Year and was given an honorable mention as Nebraska’s Nurse of the Year. In December 2011, she was named one of the top 40 professionals under the age of 40 in Omaha, Neb.
Helen was the keynote speaker for the Midlands Chapter Alzheimer’s Association in April 2015. She has presented for many AHCA/NCAL state affiliates on quality in assisted living and dementia related care topics, and served as a keynote speaker for the Iowa Assisted Living Association in 2017.
Director of Health Services
Trista joins Pemberly Place as the Director of Health Services. She brings to our residents her prestigious national certification in gerontological nursing. The American Association of Nurse Credentialing endorses this program and has documented numerous benefits to the residents when receiving care by a board-certified nurse.
She has a passion for working with the elderly and continually strives to improve the care for residents. She brings her past experiences in leadership roles in Long Term care and rehab to provide quality care to residents, family and friends alike. In Trista’s previous position as, Clinical Care Coordinator being promoted from her role as Resident Assessment Coordinator at a post-acute rehabilitation facility, Trista oversaw many aspects of team-member development and direct patient care. In this previous role, Trista was a key member of opening this brand-new facility and an integral component of its continued success. Trista is committed to the public outreach of her organization. She often gives presentations to staff at other facilities about infection prevention, OSHA regulations and Blood Borne Pathogens.
In another previous role, Trista served as Minimum Data Set Coordinator assisting residents and their family members with care-planning. Within this role, she developed and conducted education training for team members to reduce the spread of facility-related infections, increase team member and resident immunization rates within the community. In this role, she played an integral part of the reduction of antipsychotics in those with cognitive impairments.
This along with becoming a Certified Dementia Practitioner in 2015, she has gained the skills and experience to provide high-quality care to residents and reassure family members during times of transition from home to Pemberly Place.
